Declaration & Membership Agreement
REVIVE WELLNESS PMA
I, the undersigned, hereby make this Declaration and enter into this Membership Agreement with Revive Wellness PMA (the "Association").
DECLARATION OF UNDERSTANDING
I declare that I understand and acknowledge:
1. PRIVATE MEMBERSHIP ASSOCIATION: This Association operates as a private membership association under the rights of freedom of association, speech, and assembly guaranteed by the First Amendment to the Constitution of the United States.
2. EDUCATIONAL PURPOSE: The information, products, and services offered through this Association are for educational purposes only and are not intended to diagnose, treat, cure, or prevent any disease.
3. PERSONAL RESPONSIBILITY: I take full responsibility for my health decisions and understand that I should consult with qualified healthcare practitioners regarding my health concerns.
4. NOT MEDICAL ADVICE: Nothing provided through this Association constitutes medical advice, and I understand that the Association and its members do not practice medicine or provide medical services.
5. PRIVATE DOMAIN: All activities within this Association occur in the private domain, not subject to public regulation.
MEMBERSHIP AGREEMENT
By accepting membership, I agree to:
1. Respect the private nature of this Association and its members
2. Use all information and products responsibly
3. Hold harmless the Association and its agents from any claims
4. Maintain confidentiality of private Association matters
5. Comply with all Association guidelines and terms
I make this Declaration freely and voluntarily, without coercion, and I affirm that I am of sound mind and legal age to enter into this agreement.

About us
Revive Wellness PMA was born out of a deeply personal battle — one that our own family walked through with Lyme disease. I was diagnosed first, but it didn’t stop there.
Three of our children would eventually be affected as well. The journey was overwhelming — filled with confusion, misdiagnoses, fear, and frustration at the limitations of conventional medicine.
We tried everything — long courses of antibiotics, herbal protocols, essential oils, parasite cleanses, IV therapies, even Rife machines. But nothing brought full healing. We later learned why: many of these remedies couldn’t reach the brain, where Lyme and co-infections often hide. They simply couldn’t cross the blood-brain barrier.
